EMERGENCY MEDICAL TREATMENT
I authorize Hydropunk LLC to seek emergency medical treatment on my behalf if circumstances reasonably require it.

PROPERTY DAMAGE
I accept responsibility for my personal property.
Hydropunk LLC shall not be responsible for theft, loss, damage, destruction, or disappearance of personal belongings.

PHOTOGRAPHY AND MEDIA RELEASE
I grant Hydropunk LLC permission to photograph, record, film, livestream, reproduce, publish, distribute, and use my image, voice, likeness, statements, and participation for educational, promotional, documentary, historical, fundraising, commercial, and organizational purposes.
I understand that no compensation is owed for such use.
I waive any rights to inspect or approve finished materials.
